Complete & Post the OSHA 300A Summary for 2015 by February 1

Just a reminder. The OSHA 300A summary form for recordable workplace injuries and illnesses must be completed each January and posted in the workplace for all of February, March and April. You will need the following items to complete the form: The OSHA 300 log of injuries/illnesses for last year, the total manhours worked last year, and the number of employees at the company last year.

This year companies will be required to upload the OSHA 300A summary information to OSHA (during March - June). You should expect your company injury & illness data for 2016 to be posted on-line (with company name) in late 2017. As always, companies should take care to include all cases considered as "recordable" by OSHA. However, it's now important to not over-report by including minor injuries where only simple first-aid was administered and the worker was sent back to work the next morning.

Example cases considered as "OSHA Recordable" work-related injury or illness:

  • Clinic paperwork calls for "days-away from work" or "with work restrictions", or a temporary "job transfer"
  • The treatment involved stitches, a drug prescription, an amputation, loss of eye, loss of consciousness, or admittance to a hospital.
  • Note: This is also the case when the worker works for a temp agency but is supervised by your company (The incident goes on the manufacturing company OSHA 300 log, regardless of whether it is also on the Temp company's log). 

Example cases considered "Not OSHA Recordable":

  • The worker was given basic bandages, eye flushing, and/or told to take an Advil; and the clinic paperwork says "return to work with no restrictions".
  • Note: If a temp worker has an "OSHA Recordable" injury or illness and they are supervised throughout the day by an on-site supervisor who also works for the temp company, this case should only be listed on the temp services company OSHA 300 log (It should not be included on the manufacturing company OSHA log where the accident occurred).
